CarePartners is seeking a skilled and compassionate Registered Nurse Home Care PACE Specialist to join our team. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals in their homes.
Responsibilities:
- Assess patients' physical, psychosocial, and environmental needs, and develop individualized care plans to address these needs.
- Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
- Collaborate with the interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care and achieve patient goals.
- Supervise and provide clinical direction to home health aides and licensed practical nurses (LPNs) to ensure quality and continuity of services.
- Participate in on-call rotation and emergency call as needed.
Requirements:
- Current licensure as a Registered Nurse in the state of North Carolina.
- Basic Cardiac Life Support (BCLS) certification.
- Associate's degree in nursing or higher.
- Valid driver's license.
