Licensed Practical Nurse Wellness Coach

Our objective is to facilitate a transition towards preventative healthcare through our exceptional preventative care platform.
Your Role in Our Purpose:
As a Wellness Coach, you will engage remotely for 20-25 hours each week alongside a team of nursing professionals to oversee patients with chronic illnesses participating in Medicare's Chronic Care Management initiative.
Your Daily Responsibilities Include:
- Instructing patients on self-management techniques and establishing personal health goals. Chronic conditions may include: Diabetes, Congestive Heart Failure, Chronic Obstructive Pulmonary Disease/Asthma, Hypertension, Coronary Artery Disease, Ischemic Heart Disease, Anxiety, and Depression.
- Enhancing and updating the Care Plan by modifying medications, scheduling appointments, documenting biometrics, vital signs, and coaching interventions.
- Employing Motivational Interviewing or similar behavioral change strategies to guide and support patients in self-management.
- Executing Transitional Care Management tasks for high-risk patients recently discharged from hospitals or emergency rooms to minimize avoidable readmissions, which includes medication reconciliation, ensuring medication adherence, identifying warning signs, addressing obstacles, promoting follow-up care, and advising on appropriate care levels.
- Bridging care gaps by motivating or assisting with preventive services and chronic care management, such as annual wellness visits, follow-up appointments, medication oversight, pre-visit laboratory tests, necessary diagnostic evaluations, and preventive cancer screenings.
- Connecting patients with community resources as necessary, including transportation, personal care assistance, homemaker services, and social support.
Essential Skills and Competencies:
- Proficient in English.
- Strong communication abilities, self-motivated, capable of working independently with minimal supervision while achieving performance targets.
- Genuine passion for nursing.
- Meticulous attention to detail.
- Outstanding organizational and time management capabilities.
- Excellent communication and telephonic skills.
- Robust critical thinking and problem-solving abilities.
- Commitment to a consistent number of hours each day and specific days of the week.
- Availability for calls on weekdays during standard business hours.
- LPN must possess a reliable internet-connected computer.
- Current, unrestricted Compact LPN license.
- Familiarity with electronic health records and web-based applications.
- Minimum of 5 years of experience as a Licensed Practical Nurse.
- Experience in Case Management or Chronic Disease Management.
- Certification in Case Management.
- Certification as a Diabetes Educator.
- Experience in Transitional Care Management.
- Familiarity with Motivational Interviewing or other behavior change communication techniques.
Payment Structure:
This position operates as a 1099 contract role with no predetermined end date. Care Coaches are responsible for their own tax obligations and insurance coverage.
Compensation is structured at $10.00 per initial clinical interaction per patient per month. A clinical interaction is defined as occurring after two criteria are satisfied: a successful clinical call with a patient and a minimum of 20 minutes recorded in their chart timer. For example, if you successfully contact and engage two patients for 20 minutes each within one hour, your compensation for that hour would total $20.00 ($10.00 per patient x 2).
- In addition to successful clinical interactions, Care Coaches will receive $3.00 for each patient who withdraws from the Chronic Care Management Program.
- Furthermore, a payment of $4.00 will be issued after five unsuccessful attempts to reach a patient without receiving a response.
