Key Responsibilities:
As a Community Outreach Counselor, you will play a crucial role in delivering a wide array of crisis support services to our clients. Your responsibilities will include:
- Conducting outreach to individuals discharged from inpatient facilities on the day of their release.
- Meeting with clients within seven (7) days post-discharge to ensure continuity of care.
- Providing counseling and clinical support to address gaps in care.
- Assisting clients who require inpatient care and coordinating referrals as necessary.
- Delivering care in inpatient settings to facilitate a smooth transition back to outpatient services.
- Performing screenings, assessments, and evaluations for patients in crisis, whether on-site, via phone, or in the community.
- Assessing the severity of crisis situations and documenting interactions accurately.
- Collaborating with emergency response systems and other relevant services.
Qualifications:
To be considered for this role, candidates should possess:
- A bachelor's or master's degree in a relevant social service field.
- A minimum of one (1) year of direct counseling experience or relevant internship.
- Certification from the Pennsylvania Certification Board (PCB) or the ability to obtain it within one year of employment.
- A valid Pennsylvania Driver's License and the capability to provide community-oriented services.
- The ability to work 40 hours per week during agency operating hours.
Physical Requirements:
Applicants must be able to:
- See well enough to read reports and operate a vehicle.
- Communicate effectively with clients and colleagues.
- Move about the agency and community as needed.
- Lift up to 5 pounds.
- Utilize technology for documentation and communication.
Preferred Qualifications:
Ideal candidates will have:
- A master's degree in a related field with at least three (3) months of direct counseling experience.
- Licensure as LSW, LCSW, or LPC with experience in substance use or mental health counseling.
- Bilingual proficiency in English and Spanish.
- Knowledge of trauma-informed care approaches.
- Familiarity with community resources for low-income, high-risk populations.
