Home Health Physical Therapist Assistant

We are seeking a skilled Home Health Physical Therapist Assistant to join our team at LHC Group. As a key member of our healthcare team, you will provide high-quality patient care and education in the comfort of patients' homes in Clifton, New Jersey.
Job Summary
The Home Health Physical Therapist Assistant will work under the supervision of a licensed Physical Therapist to provide treatment and patient education aimed at improving patients' well-being. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Responsibilities
- Provide treatment and patient education to patients in their homes, following established plans of care.
- Instruct and aid patients in active and passive exercise, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training.
- Observe, record, and report patients' responses to treatment and changes in their condition to the supervising Physical Therapist, nurse supervisor, and/or physician.
- Coordinate care with other healthcare team members as needed.
- Instruct patients, family members, caregivers, and other healthcare team members in therapy areas within the scope of the Physical Therapist Assistant.
Requirements
- Current licensure, registration, or certification to assist in the practice of physical therapy in the state of New Jersey.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
