- Provide direct patient care to individuals with rehabilitation needs, including assessment, planning, implementation, and evaluation of care.
- Develop and implement individualized care plans that address the unique needs and goals of each patient.
- Collaborate with interdisciplinary teams, including physicians, therapists, and other healthcare professionals, to ensure comprehensive and coordinated care.
- Conduct thorough assessments of patients' physical, emotional, and social needs, and develop plans to address these needs.

Requirements:
- Current licensure as a Registered Nurse (RN) in the state of practice.

- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
