Job Summary:

Responsibilities:
  • Coordinate the multidisciplinary care delivered to patients, optimizing patient outcomes and satisfaction.
  • Review patient medical records to diagnose, treat, and provide care management and education to patients and families.
  • Makes appropriate patient referrals and monitors national health guidelines for patient care management.
  • Provides evidence-based and compassionate care to all patients, improving patient outcomes and satisfaction scores.
  • Maintains a collaborative environment with other healthcare professionals to provide quality patient care.
  • Supports academic, educational, and research endeavors, supervising Advanced Practice Professionals as needed.
Requirements:
  • Current Medical Doctor (M.D) or Doctor of Osteopathy (D.O.) degree from an accredited school of medicine.
  • Residency and/or Fellowship training in Family Medicine.
  • Current, unrestricted medical license to practice in the State of Texas.
  • Board Certification or Board Eligible.
  • Unrestricted work authorization to work in the United States.
