Pharmacist - Patient Care Specialist

About the Role:
The Pharmacist will play a vital role in ensuring the safe and effective dispensing of medications to our patients. This includes providing key information such as side effects, contraindications with other medicines, and other concerns.
Key Responsibilities:
- Work with the pharmacy manager to meet area-specific goals and departmental objectives.
- Establish collaborative pharmacist-patient relationships to provide personalized care.
- Participate in training and development for other pharmacists, technicians, and interns to enhance skills and knowledge.
- Ensure the safe and accurate dispensing of medications, including controlled substances in accordance with MA and Federal laws.
- Provide accurate verification of medications and identify, mitigate, prevent, and document problems with prescriptions.
- Offer counseling to patients on the proper use of their medications, possible side effects, and the role of prescribed medications in the overall therapeutic care plan.
- Manage all queues in the pharmacy software system and participate in risk evaluation and mitigation strategy (REMS) compliance and monitoring.
- Utilize the Mass PAT prescription monitoring program for appropriate controlled substance monitoring and provide Medication Therapy Management.
- Assist providers with pre-authorization and the use of Cover My Meds, if applicable.
Requirements:
- BS Degree in Pharmacy Required
- PharmD Degree Helpful
- Valid license to practice as a pharmacist in MA Required
- Obtain CE credits needed for license renewal as required
- Previous experience preferred
