Job Summary

We are seeking a skilled and compassionate Healthcare Case Manager to join our team at Mass General Brigham in Wellesley Hills, Massachusetts.

About the Role

The ideal candidate will have experience in general acute care hospital experience, healthcare reimbursement, quality improvement, and enhanced models of healthcare delivery. As a key member of our team, you will be responsible for facilitating discharge plans, coordinating post-acute care services, and collaborating with healthcare providers to ensure seamless transitions of care.

Responsibilities
  • Facilitate discharge planning, including coordination of post-acute care services for patients admitted to Medical, Surgical, Pediatric, or Obstetrical Services.
  • Develop and implement discharge plans for patients identified as needing post-acute discharge services or post-acute follow-up.
  • Collaborate with healthcare providers, insurers, and community-based organizations to ensure continuity of care.
  • Routinely review patients' health records and communicate with healthcare teams to ensure continued appropriate level of care.
Requirements
  • Graduate of an accredited school of nursing with BSN preferred.
  • Current licensure as a Registered Nurse in the Commonwealth of Massachusetts with minimum three years' clinical experience.
  • Evidence of continued education and professional development, including CCM or ACMA certification preferred.
