LHC Group is seeking a skilled Physical Therapist to join our team in Burnham, Pennsylvania. As a Home Health Physical Therapist, you will play a vital role in providing high-quality, patient-centered care to individuals in their homes.

About the Role

As a Physical Therapist with LHC Group, you will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. You will work closely with the patient, their family, and the healthcare team to develop a personalized treatment plan, perform interventions, and evaluate progress.

Responsibilities
  • Evaluate patient functional status and physical therapy needs, assisting the physician in developing the therapy plan of care.
  • Instruct and inform participating members of the healthcare team, the patient, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
Requirements
  • Current Physical Therapy licensure in the state of Pennsylvania.
  • Ability to work independently and as part of a team.
