As a RN Case Manager, you will be responsible for providing and supervising primary care to patients in their home setting, ensuring that their physical, emotional, and educational needs are met.
Key Responsibilities:- Assess, coordinate, and manage patients' total needs, following physicians' orders, using the nursing process.
- Develop and implement patients' plans of care, coordinating with other disciplines to ensure goals are met.
- Supervise LPNs and Home Health Aides throughout the continuum of care.
- Provide patient/caregiver education to develop and encourage self-care to achieve positive outcomes.
- Communicate patient status changes and lab results to the physician as appropriate.
- Graduate of an approved school of professional nursing.
- Reg Nurse (Single State) or RN - Multi-state Compact.
- Basic Life Support or BLS - Instructor.
- Minimum 1 year of medical/surgical nursing experience, with Home Health and ICU experience preferred.
- A dynamic and supportive work environment.
