Care Manager

We are seeking a highly motivated and compassionate Care Manager to join our team at Greater Mental Health of New York. As a Care Manager, you will play a crucial role in coordinating community-based care management services for children ranging from birth to 21 years of age. Your primary responsibility will be to provide outreach and engagement to children and families, conduct comprehensive assessments, and develop individualized plans of care in collaboration with each family and their care team.
Key Responsibilities- Provide outreach and engagement to children and families to assess their needs and develop individualized plans of care.
- Conduct comprehensive assessments of children and families to identify their strengths and challenges.
- Develop and implement individualized plans of care in collaboration with each family and their care team.
- Assist families in securing identified services and advocating to other systems for the provision of these services.
- Facilitate the creation of a support network for each child and family, convening regular meetings with the family and care team to monitor progress towards jointly developed goals.
- Provide crisis intervention and stabilization to children and families as needed.
- Develop knowledge of and positive relationships with the multiple systems serving families and youth in Westchester County.
- Provide 24-hour telephonic crisis intervention services on a rotating basis.
- Document client progress and maintain a permanent record of client activity according to established methods and procedures.
- Practice with a person-centered, trauma-informed philosophy in alignment with the values of Greater Mental Health.
- Bachelor's Degree with two years of relevant experience, or licensed as a Registered Nurse with two years of relevant experience, or a Master's Degree with one year of relevant experience.
- Basic computer knowledge is required.
- Reliability and the ability to work independently, with a strong understanding of the importance of maintaining confidentiality.
- Possession and maintenance of a valid Driver's License with a satisfactory driving record according to our insurance carrier's standards.
- Must maintain current NYS Defensive Driving certification within 30 days of hire.
- Must maintain pre-employment clearances by applicable regulatory clearances as required.
- Legal eligibility to work in the United States.
- Salary Range: $44K- $50K, based on departmental budget, qualifications, experience, education, and licenses.
- Flexible Work Arrangements: Subject to business needs, including hybrid options.
- Paid Time Off: 25 days of PTO, 12 sick days, and 11 company-paid holidays per year (prorated for part-time staff).
