We are seeking an experienced Registered Nurse (RN) for a Case Management Utilization position in our Emergency Department.

Estimated Salary Range:

$120,000 - $160,000 per year

Job Description

In this role, you will work closely with physicians, other healthcare professionals, and patients to develop, implement, and evaluate individualized plans of care. You will also coordinate discharge planning, transfer coordination, and utilization management to ensure seamless transitions of care.

As a Case Manager, your responsibilities will include:

  • Making recommendations to physicians for alternate levels of care
  • Functioning as an indirect caregiver, patient advocate, and managing patients in the most cost-effective way without compromising quality
  • Collaborating with physicians and other healthcare professionals to develop, implement, and document individualized plans of care
Required Skills and Qualifications

To be successful in this role, you will need:

  • Two (2) years combined RN experience in an acute care setting or case management
  • A working knowledge of regulatory requirements and accreditation standards (TJC, Medicare, Medi-Cal, etc.)
  • Demonstrated ability in planning, organizing, conflict resolution, and negotiating skills
  • Computer literacy skills
  • A Bachelors degree in nursing or a healthcare-related field

Additionally, preferred qualifications include acute case management hospital experience.
