Home Care LPN Job Description

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Interim HealthCare of the Twin Cities. As a Home Care LPN, you will have the opportunity to make a meaningful difference in the lives of our patients and their families.

Key Responsibilities:
  • Participate in ongoing interdisciplinary assessment of the patient.
  • Develop and evaluate the plan of care in partnership with the patient, representative (if any), and caregiver(s).
  • Provide services that are ordered by the physician as indicated in the plan of care.
  • Recognize and report changes in the patient's condition to the RN supervisor.
  • Counsel and educate the patient and family in meeting healthcare related needs.
  • Complete timely and accurate clinical notes including addressing patient's progress.
Requirements:
  • Current license as a Licensed Practical Nurse (LPN) in the State of MN.
  • 2 years' experience as an LPN.
  • Able to travel within geographic area serviced by the office.
  • Valid driver's license, reliable transportation, and proof of auto insurance.
  • CPR Certification.
