We are seeking a Specialty Pharmacist to contribute significantly to our mission at AndHealth.

Key Responsibilities:

  • Deliver outstanding patient care and support to healthcare providers at the health center.
  • Provide clinical pharmacy services as part of the Patient Management Program.
  • When applicable, deliver specific clinical services under a protocol or collaborative practice agreement.
  • Offer clinical services, therapeutic advice, and counseling to patients managing chronic conditions such as asthma, dermatitis, epilepsy, hyperlipidemia, infertility, Hepatitis B, Hepatitis C, HIV/AIDS, Inflammatory Bowel Disease (IBD), Lupus, Migraine, multiple sclerosis, psoriasis, psoriatic arthritis, pulmonary arterial hypertension, rheumatoid arthritis, and transplant.
  • Regularly review and stay informed about clinical guidelines and literature essential for effective specialty care management.
  • Serve as a resource for providers and patients seeking health and medication information.
  • Conduct thorough reviews of patient therapies for potential duplications, drug interactions, and allergies.
  • Actively engage in the pharmacy's Quality Management Committee and Quality Management Program.
  • Report on Quality Improvement Projects across pharmacy locations.
  • Evaluate and suggest medication alternatives, consolidation, and dosage adjustments in line with updated clinical guidelines.
  • Collaborate closely with Engagement Specialists to ensure continuous medication access for all patients.
  • Review insurance formularies and manage prior authorizations and appeals as needed.
  • Document all required information in specialty pharmacy software to maintain accreditation standards.
  • Participate in the Quality Management Committee as necessary.
  • Engage in URAC and ACHC specialty pharmacy accreditation preparations and visits as required.
  • Mentor pharmacy interns and students.
  • Perform any additional duties as assigned.

Education & Licensure Requirements:

  • PharmD, BS in Pharmacy, or equivalent FPGEC (NABP).
  • Unrestricted pharmacist license in good standing in the state of Ohio.
  • Willingness to obtain and maintain licenses in multiple states.

Preferred Skills and Qualifications:

  • Minimum of 3 years of direct patient care experience in pharmacy.
  • BLS/CPR certification required (or must be obtained within one month of employment).
  • Immunization delivery certification preferred.
  • At least 2 years of specialty pharmacy experience preferred.
  • Completion of a PGY1 residency or equivalent work experience preferred.
  • CSP credential preferred.
  • Exceptional written and verbal communication skills.
  • Strong attention to detail and ability to manage multiple tasks simultaneously.
  • Comfortable with technology and adaptable to new systems.
  • Willingness to travel and possess reliable transportation.
