Position Overview

SUMMARY:

  • Engages in the continuous care of patients, which includes overseeing medication, providing health education, conducting assessments, managing cases, and delivering therapeutic services.
  • Facilitates staff training and orientation, aids in team oversight, and contributes to the design, development, and execution of services.

KEY RESPONSIBILITIES:

  • Administers prescribed medications as instructed by healthcare professionals and observes their effects.
  • Offers guidance and education regarding medication effects, potential side effects, and healthy lifestyle practices.
  • Keeps patient records and charts in accordance with established protocols and standards.
  • Delivers case management support for patients, including the formulation of treatment and discharge strategies, follow-up care, and case closure.
  • Provides outpatient clinical services, including crisis intervention, to individuals, families, and groups.
  • Conducts physical assessments, screens clients who walk in, and assists physicians during physical examinations.
  • Collects laboratory samples, reviews lab results, and coordinates follow-up medical care.
  • Participates in outreach and advocacy efforts for clients.
  • Attends and engages in team meetings, staff discussions, and training sessions.
  • Establishes and maintains collaborative relationships with other healthcare organizations in the community.
  • Offers training and orientation to fellow staff members.
  • Contributes to the planning, development, and execution of new services aimed at underserved populations.
  • May serve as unit supervisor in the absence of leadership or provide on-site supervision of clinical and administrative staff.
  • Supports behavioral health providers as necessary.
  • Coordinates comprehensive care and services to promote optimal physical and mental health for individuals served within a trauma-informed care framework.

QUALIFICATIONS:

  • Must possess a Registered Nurse license from the State of Arizona.

Knowledge, Skills, and Abilities:

  • Extensive knowledge of nursing principles and practices, particularly in community and preventive mental health.
  • Proven ability to deliver comprehensive nursing care, psychotherapy, and crisis intervention.
  • Capability to establish and maintain compassionate relationships with patients and their families.
  • Ability to provide necessary in-service training.
  • Some familiarity with administrative, management, and supervisory practices.
  • Ability to work effectively within a multidisciplinary team and engage with the public.
  • Understanding of the cultural characteristics of diverse minority groups.
  • Effective communication skills, both verbal and written.
