About the RoleWe're seeking an experienced Advanced Practice Provider to join our team in providing high-quality, home-based primary care to our elderly patients. As a member of our team, you'll have the opportunity to work autonomously, develop strong relationships with patients and families, and contribute to the growth and success of our organization.
Key Responsibilities- Provide comprehensive, patient-centered care to elderly patients in their homes
- Develop and implement personalized care plans to meet the unique needs of each patient
- Collaborate with other healthcare professionals to ensure seamless care coordination
- Stay up-to-date with the latest clinical guidelines and best practices in geriatric care
- A competitive compensation package, including a base salary and bonus structure

- National Board Certification as an Advanced Practice Provider
- Licensure to practice in the Commonwealth of Massachusetts
- Graduate of an accredited Nurse Practitioner program, Master's level preferred
- Minimum two years of experience as an Advanced Practice Provider, with a focus on geriatric care
