We are seeking a skilled Dietitian-HYBRID to join our team at Nutrition that Works, LLC. As a consultant dietitian, you will work with our Long Term Care facility in Hurricane, WV, providing 16-24 hours of service per week. This is a hybrid remote opportunity, allowing for flexibility and work-life balance.

Key Responsibilities:
  • Conduct clinical nutrition assessments to ensure optimal patient care
  • Document patient information using the Nutrition Care Process in our Electronic Medical Record (EMR) system
  • Collaborate with the interdisciplinary care team to develop comprehensive care plans
  • Complete the Minimum Data Set (MDS) - Section K, and calculate Total Fluid (TF)
  • Provide recommendations for wound care and significant weight changes
Requirements:
  • Credentialing by the Commission on Dietetic Registration (CDR)
  • Dietetic licensure
  • Positive attitude and client-centered approach
  • Excellent communication skills
