Position Overview:


The Admission RN evaluates patients in their homes or hospitals and initiates care within the Chapters Health System. This role ensures a smooth transition to Care Navigation and acts as a representative of the organization.


Qualifications:

  • Bachelor's degree in nursing or a related field, or an equivalent combination of education and experience
  • Active Florida RN license, or a current multistate/compact nurse license
  • At least two (2) years of RN experience in acute care, hospice, home health care, or similar settings
  • Strong clinical assessment skills, preferably with experience in emergency or intensive care
  • Familiarity with EMR/EHR systems
  • Proficient communication skills in English; bilingual in Spanish is a plus
  • Professional presentation of services
  • Valid driver's license and automobile insurance as per company policy
  • Reliable transportation
  • Team-oriented with the ability to manage multiple priorities
  • Capability to perform essential job functions with or without reasonable accommodation
  • Strong focus on goals and time management
  • Excellent customer service skills
  • Current BLS certification for healthcare professionals

  • Explain services to potential patients and families, addressing concerns with empathy.
  • Identify and respond to admission objections and barriers.
  • Educate patients and families on admission requirements.
  • Complete informed consents and initial assessments according to regulations.
  • Collaborate with physicians to initiate care plans.
  • Implement nursing interventions to enhance patient comfort.
  • Obtain necessary physician orders.
  • Document all actions in accordance with company policy.
  • Act as a representative at assigned facilities, facilitating referrals.
  • Maintain communication with Care Navigation staff for timely follow-up.
  • Provide necessary clinical communication promptly.
  • Discuss potential needs with after-hours staff.
  • Ensure safe discharge for referred patients while meeting productivity standards.
  • Gather medical records for referrals.
  • Build strong relationships with case managers and physicians.
  • Collaborate with Professional Relations Representatives to educate on services.
  • Provide care for patients in a hospital setting as needed.
  • Perform additional duties as assigned.

Physical Requirements:

This position requires the ability to see, hear, talk, walk, and use hands for various tasks.


Frequently required to:
stand, sit, reach, lift, bend, and balance.
