Wraparound Facilitator

We are seeking a dedicated and compassionate Wraparound Facilitator to join our team at Meridian Health Services. As a key member of our team, you will play a vital role in providing whole-person healthcare to individuals in need.

Key Responsibilities:

• Conduct comprehensive assessments and reassessments to monitor progress and identify areas for improvement.
• Guide the transition of youth from a Psychiatric Residential Treatment Facility (PRTF) to the community.
• Engage with families and teams to explore and assess strengths and needs.
• Facilitate and coordinate family and team meetings to ensure effective communication and collaboration.
• Develop and implement crisis plans to ensure the safety and well-being of individuals.
• Assign tasks and responsibilities to team members to ensure efficient and effective service delivery.
• Develop and maintain a written Plan of Care that is approved by the Division of Mental Health and Addiction.
• Reassess and amend the Plan of Care as needed to ensure ongoing approval.

Requirements:

• Bachelor's degree in a relevant field, such as Social Work, Sociology, Psychology, or Education.
• Two years of experience working with Severe Emotional Disturbance (SED) youth.
