Nurse Practitioner Home Health

We are seeking a skilled and compassionate Registered Nurse to join our Home Health team at LHC Group. As a Registered Nurse, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to achieve optimal patient outcomes.
Responsibilities:- Conduct comprehensive assessments of patients to identify their unique needs and develop personalized care plans.
- Provide direct patient care, including administering medications, wound care, and other treatments as prescribed by physicians.
- Collaborate with patients, families, and caregivers to educate them on disease management, medication adherence, and self-care techniques.
- Communicate effectively with healthcare providers, insurance companies, and other stakeholders to ensure seamless care coordination.
- Document patient progress, identify areas for improvement, and adjust care plans as needed.
- Current RN licensure in the state of practice.
- Current CPR certification.
- Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
