The LPN MDS Specialist plays a crucial role in the Resident Assessment Process by completing relevant items on the Minimum Data Set (MDS) and the associated Care Area Assessments (CAAs) to ensure optimal patient care and appropriate nursing center reimbursement.
Position Highlights:
*Lead the nursing center's interdisciplinary Resident Assessment Instrument (RAI) process, ensuring compliance with guidelines for completion, computer encoding, and electronic state transmission of the MDS.
*Contribute to the development of personalized resident Care Plans, identifying resident needs and anticipated outcomes, as mandated by federal and state regulations.
*Educate new interdisciplinary team members on clinical content and the electronic completion of relevant MDS items.

Restrictions may apply based on collective bargaining agreements, applicable state laws, and factors such as employment classification (full-time, part-time, or casual), job grade, location, and tenure.
Qualifications:
*Must hold a current LPN license in good standing with the state. *Prior experience in a hospital, long-term care facility, or other healthcare environments is essential. *Knowledge of general, rehabilitative, and restorative nursing practices, as well as laws and regulations governing long-term care, is required. *Proficiency in computer applications, including Windows, is necessary. *Experience with AccuMed is preferred. *Previous experience as an MDS Coordinator is advantageous. *Experience in completing MDSs and Care Plans is mandatory. *Demonstrated expertise in RAI/PPS processes is required. *Must maintain a current BLS/CPR certification (excluding ALFs/ILFs).
