Job SummaryWe are seeking an Advanced Practice Clinician to join our team of committed healthcare professionals. As an APC at knownwell, you will work alongside a team of physicians, APCs, registered dietitians, registered nurses, and registered pharmacists to provide comprehensive medical care with a focus on metabolic health and obesity-related conditions.
Responsibilities- Practice exceptional medicine within knownwell, seeing adults for primary care and metabolic health, and adolescents ages 12+ for metabolic health and primary care.
- Embodies the knownwell mission and values of patient inclusivity in how you deliver care.
- Coordinates with the broader care team to ensure patients have the support they need.
- Documents as necessary to ensure accurate billing, including review of remote patient monitoring data.
- Communicates with patients in a timely manner through our portal for questions and concerns that have been pre-screened to necessitate a clinician response.
- Escalate any concerns to your dedicated site leadership team.
- Provide clinical guidance to medical assistants and nursing teams as assigned.
- Stay current with licensure and appropriate re-certifications.
- Master's degree (MSN) and certification by one of the Nurse Practitioner certifying bodies (ANCC or AANPCB) as a family nurse practitioner, or Master's degree in Physician Assistant studies and certification by the Physician Assistant certifying body (NCCPA).
- Unrestricted DEA licensure and prescribe authority (per state practice regulation).
- At least 2 years of clinical experience in an ambulatory care setting, family or internal medicine; experience in chronic condition management and weight management preferred.
- Active, unrestricted license to practice in Massachusetts, or willingness to obtain.
- Excellent communication and interpersonal skills.
- Dedicated to staying current with the latest developments in primary care and obesity medicine.
- Believe in inclusive, compassionate care.
- Believe in shared decision making with a patient.
- High emotional intelligence and ability to make patients feel at ease without judgment or shame.
