Social Services Professional

The Social Worker provides social services to patients and families in both complicated and uncomplicated situations, utilizing experience, knowledge, judgment, and skills to promote patient rights and confidentiality.
Key Responsibilities- Maintains patient confidentiality and promotes patient rights as supported by agency, state, and federal directives.
- Identifies cultural diversity and lifestyle choices and accommodates for such differences in a non-judgmental manner.
- Provides consultation to other hospice staff and volunteers while consistently demonstrating the Hospice Savannah values and compliance with organization's service excellence standards.
- Models the Hospice Savannah Corporate Values: Excellence, Accountability, Compassion, Teamwork, and Stewardship.
- Adheres to and promotes the ethics and values of the Social Work profession using the NASW Code of Ethics as a guide to ethical decision making in social services practice.
- Graduate of an accredited school of social work with Master's degree in Social Work required, licensed Social Worker required.
- Minimum of 2 years social services experience, family work, and healthcare experience preferred.
- Valid driver's license, automobile insurance, and reliable transportation required.
- Excellent communication skills (written and verbal) and the ability to successfully interact with a diverse patient population, families, physicians, Hospice Savannah staff, and the community at-large.
- Computer proficiency in Microsoft Office, including Word and Outlook, and database knowledge required.
A full-time position works a 30-40 hour work week, rotating days, evenings, or weekends, with occasional requests to work additional days/hours/holidays as needed. The Social Worker is required to participate in the rotating on-call schedule and may be required to provide care in what has been determined by local authorities as a high-risk crime area and unsafe situations.
