Job Summary

We are seeking a highly skilled Physician Associate or Nurse Practitioner to join our team. As a post hospitalization care provider, you will work closely with our medical team to deliver exceptional care to our patients in skilled nursing facilities.

Responsibilities
  • Deliver high-quality post hospitalization care to patients in skilled nursing facilities
  • Work closely with our medical team to develop and implement individualized care plans
  • Conduct regular assessments and evaluations to ensure patient safety and well-being
  • Collaborate with other healthcare professionals to ensure seamless care transitions
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments
Requirements
  • Master's degree in a relevant field (e.g. Physician Assistant Studies, Nursing)
  • Current medical license and certification (e.g. NCCPA, AANP)
  • At least 2 years of experience in post hospitalization care or a related field
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
