**About the Job**
This is an exciting opportunity for a Primary Care Physician to join our team and contribute to our mission of improving healthcare outcomes through innovative solutions and cutting-edge technology. As a skilled clinician, you will be responsible for providing comprehensive primary care services to patients residing in SNFs, including rounding daily, prescribing medications and treatments, and collaborating with facility nursing staff to ensure seamless care transitions.
**Responsibilities and Requirements**
Your responsibilities will include:
- Providing comprehensive primary care services to patients residing in SNFs
- Rounding daily to assess patient needs and prescribe medications and treatments as necessary
- Collaborating with facility nursing staff to ensure timely documentation and effective care coordination
- Documenting patient care and encounters in our proprietary electronic health record system
- Participating in phone or secure messaging consultations with facility nursing staff as needed
You will need:
- A strong background in primary care, preferably with experience working in SNFs
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and facility staff
- Strong clinical judgment and decision-making skills, with the ability to prioritize tasks and manage multiple responsibilities
- Proficiency in using electronic health records and other relevant technology systems
**Salary and Benefits**
We offer a competitive salary range of $165,000 annually, plus benefits including paid time off, 401(k) matching, medical, dental, and vision insurance, short-term and long-term disability insurance, and malpractice insurance coverage.
