Primary Care Nurse

We are seeking a skilled and compassionate Registered Nurse to join our Primary Care team in Elkton, MD. As a key member of our team, you will provide high-quality, patient-centered care to achieve optimal outcomes.
Key Responsibilities- Provide evidence-based nursing care to patients of all ages, including pediatric, adolescent, adult, and geriatric populations.
- Participate in a relationship-based care approach to deliver care that is tailored to each patient's unique needs.
- Perform Nurse Visits, including Annual Wellness Visits, patient self-management for chronic disease education, and hospital follow-up/transitional care management calls.
- Conduct point-of-care tests, EKGs, and other tests as directed by clinicians.
- Triage walk-in patients and ensure appropriate disposition if on-site.
- Assist providers in examinations or procedures if on-site.
- Function as a liaison between patients, physicians, and staff to provide accurate communication.
- Collaborate with team members to ensure a smooth, organized, and efficient flow of patients.
- Respond to patient crisis situations.
- Update medication history.
- Assist with staff education and evaluation of clinical competencies for new and existing employees.
- Support the site by acting as the liaison for JCAHO, POCT, Tracer monitor for Safe Practice Behavior Monitoring (SPBM) tools & JCAHO Tracers.
- Administer medications, monitor vital signs, and perform routine treatments and procedures prescribed by the physician when needed.
- Keep current with policies and procedures and participate in quality improvement activities.
- Participate in special projects.
- Perform at-home visits to assess patients' needs and provide education.
- Assist patients with care coordination.
- A graduate of a state-approved or National League of Nursing accredited School of Nursing.
- RN license or eligible for licensure in the state of Maryland.
- Two years of experience as a RN, physician office experience preferred.
- BSN preferred; ADN considered with agreement of completion of BSN in 3 years.
- BLS certification required.
- Proof of a valid driver's license and active auto insurance is required.
