Job Summary

We are seeking a highly skilled Licensed Practical Nurse to join our FACT Program, a multi-disciplinary team that provides clinical and supportive services to individuals with severe and persistent mental illness.

Responsibilities
  • Provide treatment, rehabilitation, and support services to individuals in accordance with the FACT model.
  • Oversee medical services received by individuals served by the FACT Program.
  • Assure the accuracy, completeness, and confidentiality of clinical records.
  • Conduct mental health status assessments and assess physical health needs.
  • Coordinate referrals to community health care providers.
  • Ensure proper medication self-administration in conjunction with the ARNP or Psychiatrist.
  • Provide education related to medication, health needs, nutrition, and mental health issues.
  • Serve as a primary case manager for an assigned group of individuals and promote recovery, empowerment, self-determination, and decision-making for each FACT participant.
Qualifications
  • Graduated from an accredited school of Practical Nursing and a valid Florida Nursing License.
  • 6 months of experience in a medical/surgical or psychiatric care setting.
  • Ability to make sound decisions independently and interact appropriately with internal and external customers.
  • Strong communication skills and ability to contribute to the completeness and confidentiality of clinical records.
  • Meet MHRC/RBHS vehicle driver requirements, including a valid Florida Driver's License, automobile insurance coverage, and no more than eight points on their license for any combination of violations.
Position Details

Full Time Shift: Monday through Friday (rotating weekends, on-call, and holidays)
