About the Role:

Hospice Medical Social Worker - Masters Level

Amedisys is a leading provider of home health, hospice, and high-acuity care, dedicated to helping patients and families navigate the complex healthcare system.

We are seeking a compassionate and skilled Medical Social Worker to join our team in providing patient-centered care to individuals and their families in need of hospice services.

Key Responsibilities:

  • Assists patients and families with health-related financial, social, and emotional concerns.
  • Provides social work services in accordance with the plan of care.
  • Ensures timely communication of changes in patient status to healthcare team members and care center staff.
  • Educates patients, family members, and team members on medical social work interventions and treatment plans.
  • Identifies and addresses risks or threats to patient health in their environment.

Requirements:

  • Holds a Master's or Doctorate degree from a school of social work, accredited by the Council on Social Work Education.
  • Has one year of social work experience in a healthcare setting.
