The Quality Improvement (QI) Registered Nurse is tasked with developing, implementing, and analyzing all facets of quality assurance and enhancement protocols. This role is pivotal in ensuring that patients receive the highest standard of care, fostering positive outcomes while adhering to CMS regulations.
- Ensure compliance with Federal and State regulations and applicable accreditation standards in all Quality Assurance and Improvement initiatives.
- Adhere to company policies and procedures to maintain compliance.
- Establish patient-centered care plans from admission through discharge.
- Facilitate timely requests for authorizations.
- Assess the quality and effectiveness of nursing practices and services, utilizing data analysis to identify collaborative opportunities for service enhancement and improved patient outcomes.
- Participate in company committees as appointed by agency leadership.
- Support the orientation, training, and education of clinical field personnel.
- Oversee and evaluate all aspects of Quality Assurance Performance Improvement (QAPI).
- Monitor EMR Reports related to OASIS, POC, and RAP timeliness, assisting managers in developing strategies to meet deadlines.
- Act as a resource for staff regarding performance improvement across various areas, including accuracy of information and supporting documentation, with a focus on OASIS assessments and overall job performance.
- Export OASIS assessments for designated branch locations weekly and as necessary, maintaining Validation Reports of OASIS transmissions. Identify error patterns, staff performance issues, and recommend corrective actions and educational needs.
- Make decisions based on ethical principles, fostering a non-judgmental and inclusive environment sensitive to socio-cultural diversity.
- Participate in on-call duties for after-hours client care.
- Encourage a culture of quality and safe client care through active participation in the development and adherence to the QA plan and associated metrics.
- Perform additional duties and accept responsibilities as assigned.
- Current RN license.
- Minimum of five (5) years of experience in home health care.
- Comprehensive knowledge of Federal and State regulations.
- Strong organizational and communication skills.
- At least 1-2 years of experience in OASIS and home health documentation review.
- Willingness to travel approximately 25% of the time.
