Key Responsibilities:
The Clinical Assessment Nurse will oversee the RAI process and manage assessments and care plan coordination for assigned residents. Key duties include:
- Completing the MDS, CAA's, and care plans within required timelines.
- Conducting resident assessments through physical evaluations, interviews, and chart reviews.
- Collaborating with caregivers, including physicians, nursing staff, social services, therapy, dietary, and activity personnel, to discuss resident care needs.
- Reviewing information from hospitals, consultants, and external agencies to inform assessment and care planning.
- Coordinating and initiating significant changes in MDS documentation.
- Preparing for PPS meetings and maintaining MDS assessments according to Medicare schedules, as well as monitoring Medicare days and RUGs utilization.
- Staying updated with the requirements set by the American Association of Nursing Assessment Coordinators (AANAC).
Qualifications:
- Active Registered Nurse (RN) license in Michigan.
- AANC certification is advantageous; RAC-CT preferred.
- Familiarity with the Resident Assessment Instrument (RAI) process and the Prospective Payment System (PPS) is highly desirable.
- Previous experience as an MDS Nurse.
