Care Manager for Case Management at Massachusetts General Hospital

Massachusetts General Hospital, a leading healthcare institution in the United States, is seeking an experienced Care Manager to join its esteemed Case Management team. As a key member of this team, you will play a vital role in enhancing patient care services along the continuum of care.
Job Summary:
We are currently recruiting for a highly skilled and dedicated RN to work in our Case Management department. The successful candidate will have extensive experience in case management, excellent communication skills, and a passion for delivering high-quality patient care.
About the Role:
The primary responsibility of this position is to assess patients' needs, develop comprehensive care plans, and ensure seamless coordination of care across the healthcare spectrum. You will be working closely with multidisciplinary teams to provide exceptional patient outcomes, reduce costs, and ensure efficient reimbursement. Your expertise will help us improve patient satisfaction, quality of care, and overall hospital performance.
Key Responsibilities:
- Collaborate with healthcare teams to identify and address patients' complex needs
- Develop and implement individualized care plans to achieve optimal patient outcomes
- Facilitate effective communication among caregivers, families, and patients to ensure coordinated care
- Monitor and evaluate patient progress, making adjustments to care plans as needed
- Identify opportunities to enhance patient safety and satisfaction
- Stay up-to-date on the latest clinical guidelines, best practices, and regulatory requirements
Requirements:
- Bachelor's degree in Nursing (BSN) or Associate Degree in Nursing (ADN)
- 2+ years of experience in case management, preferably in an acute hospital setting
- Strong knowledge of medical-surgical nursing, disease management, and case management principles
- Excellent communication, critical thinking, and problem-solving skills
- Ability to work collaboratively with diverse stakeholders, including patients, families, physicians, and other healthcare professionals
- Proficiency in electronic medical records (EMRs) and other relevant software systems
