Physical Therapist Home Health Full Time

We are seeking a skilled Physical Therapist to join our Home Health team. As a Physical Therapist, you will play a vital role in helping patients recover from injuries, illnesses, and functional disabilities.
Key Responsibilities- Plan and administer prescribed physical therapy treatment and training for patients to attain the highest level of physical function.
- Test and screen patients' physical strengths to assist physicians in evaluating their level of function and record findings to develop or pursue treatment programs.
- Develop and implement a conditioning and rehabilitation program consistent with the physician's Plan of Treatment and the overall goals of the patient/rehab team.
- Confer with physicians and clinical team members to obtain additional patient information and assist in developing, implementing, and revising the therapy treatment program and Plan of Treatment.
- Provide Physical Therapy Assistants and Home Health Aides with written instructions and care plans that reflect the current plan of care.
- Monitor the completion of documentation by Physical Therapy Assistants and Home Health Aides as part of the supervisory/leadership responsibility.
- Accurately and thoroughly document patients' care observations, interventions, and evaluations.
- Degree from an accredited Physical Therapy Program approved by the APTA.
- Minimum of one year of physical therapy experience preferred.
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Strong organizational and communication skills.
