Outpatient Physical Therapist

Position Overview
An exceptional opportunity exists for a dedicated Outpatient Physical Therapist to join the team at FAIRFAX FAMILY PRACTICE. In this role, you will engage with motivated patients who are eager to regain their functional abilities. You will collaborate closely with physicians to develop and implement effective treatment strategies and monitor patient outcomes.

Work Schedule: Monday to Friday, 8-hour shifts
Key Responsibilities:
- Accurately documents evaluation results in accordance with departmental guidelines.
- Completes daily progress notes that reflect patient responses, performance, treatment status, and discharge requirements.
- Provides concise discharge summaries, treatment progress updates, follow-up needs, and educational information for patients and their families.
- Ensures timely and clear documentation to support reimbursement and continuity of care.
- Records evidence of patient and family education in medical records.
- Communicates effectively with healthcare team members and adapts to feedback and changes.
- Offers appropriate guidance to support staff in line with clinic policies.
- May assume leadership roles within the department or organization as needed.
Qualifications:
Certification - Basic Life Support required upon hire
Licensure - Virginia Physical Therapist license required upon hire
Experience - Preferred but not mandatory
Education - Bachelor's Degree in Physical Therapy; Master's Degree preferred
