As a key member of our healthcare team, you will provide high-quality nursing care to patients in their homes, promoting independence and dignity while ensuring the best possible health outcomes.

Responsibilities

As a Private Duty Home Health Nurse with LHC Group, you will be responsible for:

  • Providing direct nursing care as prescribed by the physician to acutely ill, and/or chronically ill patients in their homes.
  • Recording clinical and progress notes, and coordinating other patient care activities/services to provide the highest quality of patient care.
  • Working under the direct supervision of the Registered Nurse and/or designated supervisor.

Requirements

To be successful in this role, you will need:

  • A current LPN licensure in the state of practice.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
  • Current CPR certification.
  • Previous home care experience is preferred.
