Job Summary

The Hope Villages of America is seeking a skilled Case Manager to join our team. As an advocate, mentor, and life coach, you will work directly with individuals who have experienced domestic violence, providing crisis intervention, comprehensive assessments, individual and group therapy, and comprehensive case management and advocacy services.

Key Responsibilities
  • Develop and maintain individualized treatment plans for each client
  • Provide crisis intervention and trauma-informed care
  • Work with clients to plan, implement, and monitor service plans, safety plans, and relapse prevention plans
  • Manage a caseload of 20 or more clients
  • Serve as a linkage to services, including drug treatment, transitional housing, residential treatment, counseling and therapy, legal services, and job training
  • Assist clients in developing service plans addressing physical safety, stabilization, substance abuse, and mental health issues from a harm-reduction approach
  • Ensure immediate responsiveness for all referrals and crises for assigned caseload
Requirements
  • Bachelor's Degree in Social Work or related field preferred
  • 2+ years of experience in case management
  • 2+ years of experience working with individuals in two or more of the following: mental health, related issues, complex trauma, substance abuse, court involved, domestic violence, HIV/AIDS, child welfare related issues, and crisis intervention techniques
  • Knowledge of Microsoft Office Productivity Tools (MS Word, Excel, Outlook, PowerPoint)
  • Ability to work independently and possess a valid Florida Driver's License
