Position Overview:
As a Hospice RN, you will play a crucial role in delivering personalized care during pivotal moments in life.

We are committed to enhancing the quality of life for our patients through dignified and loving care. As a Hospice RN/ RN Case Manager PRN, your responsibilities will include:

  • Reporting to the Administrator, Executive Director, or Patient Care Manager Senior, while maintaining primary oversight and professional management of each patient.
  • Serving as the key liaison among the physician, patient/family, and hospice team.
  • Delivering direct patient care services to hospice patients and providing supportive care to their families, ensuring a compassionate and trusting relationship.
  • Empowering caregivers by educating them on the necessary care for their loved ones.
  • Collaborating with an interdisciplinary team, including Licensed Practical/Vocational Nurses (LPN or LVN), Certified Nursing Assistants (CNA), and support staff to ensure comfort and dignity for our patients.
  • Assessing patient and family needs and assisting in the development and implementation of individualized care plans.
  • Maintaining accurate clinical records for all assigned patients.
  • Participating in quality assessment and performance improvement initiatives.

Candidate Profile:

Essential Qualifications for Hospice Registered Nurses:

Compassionate Nature:
You should be empathetic, kind, and sensitive to the needs of patients and their families.
Comfortable providing care in various settings as per the patient's preference.
Exhibit flexibility and calmness while supporting others during challenging times.
Possess an RN license in the state of employment (or eligibility to obtain).
A minimum of 1 year of nursing experience in hospice, home health, ICU, geriatric, or oncology is strongly preferred.

Additional experience that may enhance your success in this role includes: hospital Staff RN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, or travel nursing.

Current driver's license and automobile insurance are required.
Current CPR certification is mandatory.
