Care Transition Coordinator RN

We are seeking a skilled Care Transition Coordinator RN to join our team at Christus Hospice Schumpert. As a Care Transition Coordinator RN, you will play a vital role in facilitating a seamless transition for patients discharging from a facility setting to the care of an LHC Group hospice agency.

Key Responsibilities:
  • Achieve admission goals and expectations as established at hire or at review of annual agency budget goals.
  • Assist the LHC Group agency with preparation for accepting care of the patient post-discharge from the hospital.
  • Execute contracts for facility-based services for hospice patients.
  • Explain hospice services and agency procedures to patients and their family members.
  • Involve family and caregivers in the educational process and assess post-discharge educational coaching needs.
  • Participate in bi-weekly IDG meetings to give updates on accounts, customer needs, and progress towards agency growth strategies.
Requirements:
  • One year of hospice experience or one year of hospital case management experience.
  • Current Registered Nurse (RN) licensure.
  • Excellent organizational and communication skills.
  • Thorough understanding of hospice qualifying criteria and coverage guidelines.
  • Proficient computer skills.
  • Current CPR certification, driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
