Key Responsibilities:

The Primary Care Physician (PCP) will play a crucial role in our clinical operations team, engaging in direct patient care and assessments primarily within an ambulatory healthcare setting. Responsibilities include but are not limited to:

1. Conducting comprehensive geriatric assessments, including medical history and physical examinations.
2. Diagnosing and treating various health conditions while developing personalized care plans.
3. Providing health education and facilitating specialty referrals as necessary.
4. Documenting patient interactions and care plans according to established standards.
5. Participating in clinical rounds and team meetings to ensure cohesive patient management.

Essential Qualifications:

The ideal candidate will possess:

- Current licensure in the state of practice.
- Board certification or eligibility in Internal Medicine or Family Medicine.
- A minimum of one year of clinical experience in a geriatric or family practice setting is preferred.
