About the Role:
The Pediatric Care Coordinator will work closely with community providers to ensure seamless care coordination. You will be responsible for maintaining accurate and up-to-date Medication Administration Records, scheduling needed appointments, and obtaining health-related information for clients. Your strong organizational skills and ability to follow up will enable you to effectively manage multiple tasks and responsibilities.

Responsibilities:
  • Provide primary health care management for assigned cases
  • Maintain accurate and up-to-date Medication Administration Records
  • Communicate with community providers to schedule needed appointments
  • Obtain health-related information for clients

Requirements:
  • NYS Licensed Practice Nurse required
  • Pediatric and/or Medical/Surgical experience
  • Excellent organizational skills and follow-up
  • Effective communication skills - both written and oral

What We Offer:
  • A Monday-Friday schedule with no weekends required

Estimated Salary: $62,000 - $82,000 per year
