Job SummaryThe Physical Therapist will be responsible for providing physical therapy services to patients in their own homes. This includes assessing patients' conditions, developing personalized treatment plans, and delivering therapy to help improve mobility, reduce pain, and restore function.
Responsibilities- Conduct initial and ongoing comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
- Establish and revise treatment plans in consultation with physicians as necessary.
- Assist in development and implementation of interdisciplinary client care plans as it pertains to Physical Therapy.
- Treat patients to relieve pain, develop or restore functional and maintain maximum performance, using physical means such as exercise, massage, heat, light, and electricity.
- Teach, supervise, and counsel patients and their families in the physical therapy program.
- Record all evaluation data, treatments, and patients' responses to therapy intervention in a timely manner.
- Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
- Licensed or registered by the State.
- At least one year of experience in an acute/sub-acute setting.
- Home care experience and at least one year of supervisory experience (2 years preferred).
