Job Summary:

AltaStaff is seeking a skilled Nurse Practitioner to review and make recommendations on consumer assessments for long term care services and supports. The ideal candidate will have a strong clinical background and excellent communication skills.

Key Responsibilities:
  • Review and validate assessment outcomes for long term care services and supports
  • Maintain current knowledge of regulatory changes pertaining to quality and validity of assessments
  • Assess cases presented and utilize clinical competencies to review and validate assessment outcomes
  • Work collaboratively to monitor and address clinical quality and gaps in care
  • Provide clinical and educational assistance to support project initiatives
  • Participate in performance, improvement, and quality assurance activities and related meetings
  • Meet all standards established for this position as outlined in the corresponding annual performance criteria and bonus template for this position

Requirements:
  • Current NY Nurse Practitioner Licensure
  • Proof of Board of Certification
  • Proof of Malpractice Insurance showing professional liability coverage of 1M per incident/3M agg
  • 2+ years practicing experience as a Nurse Practitioner
  • Computer literacy and the ability to navigate new and emerging technology
  • Must meet BYOD requirements
  • Highly developed written and oral communication skills; strong interpersonal skills

Additional Requirements:
  • Physical exam signed and dated by a physician within the last year
  • PPD or QuantiFeron (within 1 year) or chest X-ray with Questionnaires (every 5 years)
  • TDAP within 10 years
  • Quantitative Titers for MMR & Varicella
  • Hepatitis B vaccination schedule or Declination Form Also Accepted
  • Proof of Covid19 Vaccine
  • Proof of Flu Vaccine
