Job Summary
">Atrium Health seeks a compassionate Registered Nurse to fill the position of Post-Acute Care Coordinator. This role involves managing a caseload of patients receiving post-acute care in their homes.
You will oversee the daily management of patient care, collect data pertinent to patient health, and analyze it to determine the best course of action. Your primary focus will be on prioritizing issues and developing effective plans of care that promote quality patient outcomes and satisfaction.
Key responsibilities include:
- Managing patient caseloads and coordinating care with multi-disciplinary teams
- Collecting and analyzing patient data to identify issues and available resources
- Developing and implementing plans of care that meet patient needs and support high-quality outcomes
- Prioritizing tasks based on criticality and impact on patient health and welfare
This role requires strong communication and organizational skills, as well as the ability to lift and support weights of up to 35 pounds. The estimated salary range for this position is $80,000 - $110,000 per year.
