Nurse Practitioner

The ideal candidate will possess excellent clinical skills, a passion for value-based care, and a commitment to delivering compassionate, patient-focused care. As a key member of our care team, you will work closely with patients, caregivers, and healthcare professionals to develop and implement comprehensive care plans that address the unique needs of each patient.
Key Responsibilities:
- Assume medical responsibility for an assigned panel of patients with CKD stages 3b, 4, 5, and ESRD
- Provide clinical services focused on patient-level interventions and system-level interventions (medical and kidney-focused)
- Complete history and physical exams, order laboratory and diagnostic tests as needed, and educate patients about medication usage and side effects
- Identify and manage patients' medical conditions and obstacles to care, participate in frequent care team huddles, and educate patients and caregivers about medical conditions and treatment plans
- Order diagnostic and laboratory tests, evaluate test results, and determine the need for consultation from medical, surgical, and/or psychiatric specialties
- Facilitate care across various settings, including emergency departments, hospitals, skilled nursing facilities, home health agencies, hospices, and community-based organizations
- Perform structured assessments, develop and implement comprehensive care plans, and document all medical encounters accurately and in a timely manner
Requirements:
- Licensed Advanced Practice Nurse (APN) in the state of Florida
- Meet Duo Health clinician credentialing criteria, including passing a background check and drug screen and meeting COVID-19 vaccine requirements
- Reliable transportation, driver's license, and a verifiable good driving record
- Self-motivated and able to perform work in accordance with defined operating procedures
- Ability to develop and maintain inclusive and equitable relationships with patients, caregivers, and staff
- Experience working with community resources from diverse backgrounds
- Ability to communicate effectively both orally and in writing
