We are seeking a highly skilled Clinical Liaison Specialist to join our team of dedicated healthcare professionals. As an advocate for patients during the pre-admission process, you will work closely with multi-disciplined clinical teams to evaluate potential patients' medical needs and identify those who would benefit from our specialized extension of acute care.

Our post-ICU plan of care focuses on mobility, weight-bearing, ambulation, and nutrition, all crucial contributors to getting patients back to their baseline of health. You will travel to local referring hospitals to assist with discharge planning and help identify patients who have entered into a state of chronic critical illness.

This is a salaried position that works Monday-Friday with periodic on-call responsibility.

Key Responsibilities:
  • Evaluate potential patients' medical needs and work with clinical teams to develop effective plans of care
  • Travel to local referring hospitals to assist with discharge planning and identify patients who require extended acute care
  • Collaborate with healthcare professionals to promote patient-centered care and ensure seamless transitions between levels of care
Requirements:
  • Registered Nurse (RN) or Respiratory Therapist (RRT) License
  • Other educational and clinical designations may be considered with approval of Vice President, Business & Market Development or designee

Estimated Salary: $65,000 - $85,000 per year
