Key Responsibilities
  • Develop and implement individualized care plans for residents with orthopedic, neurological, and infectious diseases.
  • Conduct thorough assessments and evaluations to identify residents' needs and goals.
  • Provide hands-on therapy services, including exercises, stretches, and adaptive equipment training.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and support.
  • Document resident progress and outcomes accurately and timely.
  • Establish and instruct caregivers on resident-specific maintenance programs.
  • Ensure compliance with state regulations and national standards for physical therapy practice.
Requirements
  • New York State Physical Therapy Licensure
  • CPR certification
  • Physical strength to lift between 50-100lbs
  • Ability to read and understand work assignments, as well as report verbally and provide written documentation as necessary
  • Respect for residents' privacy and rights; display of genuine concern and empathy for the elderly
  • Able to perform tasks while upholding high standards of quality, cleanliness, and kindness
  • Qualities of responsibility, maturity, diplomacy, and ability to work well with residents and other staff members
