MDS Coordinator

We are seeking an experienced RN or LPN to join our team as a MDS Coordinator. This role is responsible for completing the Resident Assessment Instrument (RAI) process in accordance with state and federal laws, as well as the policies and goals of our community.

• Complete the RAI process in accordance with state and federal laws and our community's policies and goals
• Work with supervisors, co-workers, and community staff to perform duties
• Ability to work hours as scheduled based on the requirements of the position/assignment
• One year of experience preferred in working the RAI process

• Active, unencumbered Practical Nurse or Registered Nurse license in the State of practice
• Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served
• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public
