We are seeking highly motivated and compassionate Registered Nurses to join our Home Care team at UnityPoint Health. As a Home Care Nurse, you will have the opportunity to work one-on-one with patients in their homes, providing personalized care and support.

Program Highlights
  • 12-Month Residency Program: Our comprehensive program is designed to provide new graduate RNs with the skills and confidence needed to succeed in home care.
  • Clinical Experience: You will work under the guidance of an experienced preceptor, gaining hands-on experience in home care settings.
  • Ongoing Education and Development: Our program includes regular educational seminars, continuing education opportunities, and dedicated nurse educators to support your growth and development.
  • Preceptorship: You will be paired with an experienced nurse who will provide guidance, support, and mentorship throughout your residency.
  • Residency Project: You will have the opportunity to work on a project that showcases your skills and knowledge, and demonstrates your commitment to home care nursing.
Responsibilities
  • Provide high-quality patient care in the home setting, including assessments, care planning, and implementation.
  • Develop and maintain strong relationships with patients, families, and healthcare teams.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.
  • Participate in regular educational seminars and continuing education opportunities to stay up-to-date on best practices and industry developments.
Qualifications
  • Graduate of a State Board-approved program for Registered Nurses.
  • CPR certification with valid Basic Life Support (BLS) Healthcare Provider Card.
  • Strong interpersonal skills and ability to work effectively with patients, families, and healthcare teams.
  • Ability to work independently and as part of a team.
  • Valid driver's license and automobile insurance in accordance with state and/or organizational requirements.
