Occupational Therapist

Grace and Mercy Home Care Services, LLC is seeking a highly skilled Occupational Therapist to join our team. As an Occupational Therapist, you will play a vital role in helping patients recover from illness, injury, or disability. You will work closely with our rehabilitation team to develop and implement personalized therapy plans that promote patient health, independence, and overall well-being.
Responsibilities:
Develop and participate in health promotion programs, group activities, or discussions to promote patient health and social adjustment.
Plan, organize, and conduct occupational therapy to patients to help rehabilitate those impaired due to illness, injury, or psychological or developmental problems.
Consult with the rehabilitation team to select activity programs and coordinate occupational therapy with other therapeutic activities.
Test and evaluate patients' physical and mental abilities and analyze medical data to determine realistic rehabilitation goals.
Select activities that will help patients learn work and life-management skills within the limits of their mental and physical capabilities.
Plan and implement programs and social activities to help patients learn work and school skills and adjust to handicaps.
Recommend changes in patients' work or living environments consistent with their needs and capabilities.
Help patients improve decision-making, abstract reasoning, memory, sequencing, coordination, and perceptual skills using computer programs.
Provide training and supervision in therapy techniques and objectives for Home Health Aides (HHAs) to assist patients when needed.
Recommend special supplies and equipment, such as splints, braces, and computer-aided adaptive equipment.
Evaluate patients' progress and prepare reports that detail progress. Complete and maintain necessary records.
Provide patients with assistance in locating and holding jobs.
Assist Director of Patient Services to formulate company policies and procedures for physical therapy.
Qualifications:
Must possess a New York state Occupational License.
Must be a graduate of a program in occupational therapy approved by the Council on Medical Education of the American Medical Association in collaboration with the American Occupational Therapy Association, or a graduate of a curriculum in occupational therapy recognized by the World Federation of Occupational Therapists and eligible for registration with the American Occupational Therapy Association.
Must be registered with the American Occupational Therapy Association.
Preferred Skills:
Must be able to relate appropriately to, communicate effectively with, and develop trusting relationships with diverse patient populations and with staff.
Must be able to meet or exceed the Service Excellence Standards of the New York State Department of Health.
Must be able and willing to work on an as-needed basis to meet operational needs of the agency.
Being bi-lingual in Spanish/English will be a plus.
Preferably two years of experience with one year home care experience.
