Job Summary

Community Hospice of Northeast Florida is seeking a compassionate and skilled Hospice Care Aide to join our team. As a Hospice Care Aide, you will play a vital role in providing high-quality patient care and support to individuals with terminal illnesses.

Key Responsibilities
  • Perform hands-on personal care, including bathing, dressing, and grooming.
  • Assist with ambulation and exercises to promote patient mobility and comfort.
  • Provide emotional support and companionship to patients and their families.
  • Collaborate with the interdisciplinary team to develop and implement patient care plans.
  • Document patient progress and report any changes or concerns to the healthcare team.
  • Teach family members and caregivers how to provide personal care and support to patients.
Requirements
  • Current licensure as a Certified Nursing Assistant (CNA) in the state of Florida.
  • Current State of Florida Driver's license with a driving record acceptable to our insurance carrier.
  • Current CPR/BLS Certification.
  • Immunizations or screenings as required.
  • High school diploma or GED equivalent preferred.
  • 6 months of experience in caring for the terminally ill preferred.
