Position Overview

Key Responsibilities:
  • Operates under professional exemption status while maintaining a flexible schedule.
  • Interprets medication orders to ensure safety and efficacy, remaining vigilant regarding compatibility, duplication, allergies, and potential errors in administration.
  • Facilitates resolution of medication-related issues among pharmacy, nursing, and prescribers, showcasing initiative and sound judgment.
  • Monitors medication therapies for significant interactions and intervenes when necessary based on pharmacological expertise.
  • Evaluates and documents the appropriateness of selected medications to achieve desired therapeutic outcomes.
  • Communicates potential issues and solutions to prescribers effectively.
  • Acts as a consultant on pharmacokinetics and adverse reactions for healthcare professionals.
  • Participates in therapeutic drug monitoring programs, adjusting therapies based on patient conditions and lab results.
  • Processes medication orders in the Electronic Health Record system to ensure accurate documentation.
  • Maintains a detailed pharmacy patient profile, including relevant medical history and pharmacokinetic parameters.
  • Oversees the packaging, labeling, and distribution of medications according to established guidelines.
  • Monitors scheduled medication usage and maintains controlled inventories in dispensing areas.
  • Compounds and dispenses pharmaceuticals, including sterile and specialized products.
  • Verifies medication orders for accuracy prior to delivery to patient care areas.
  • Reviews laboratory data for medication appropriateness and recommends adjustments as needed.
  • Develops and presents educational programs for healthcare staff and pharmacy trainees.
  • Educates patients and families regarding their medications.
  • Responds to emergencies to assist with medication management.
  • Provides timely and accurate medication information to healthcare professionals and patients.
  • Maintains the hospital formulary and promotes the use of formulary items when suitable.
  • Documents all clinical activities and interventions performed.
  • Collects and maintains required logs and records for departmental compliance.
  • Effectively communicates professional knowledge to patients and healthcare teams.
  • Engages in quality improvement initiatives and medication use reviews.
  • Organizes and prioritizes workload effectively within time constraints.
  • Collaborates with fellow pharmacists to manage work area activities and technician assignments.
  • Reviews and resolves issues related to automated dispensing cabinets.
  • Ensures compliance with hazardous waste disposal procedures.
  • Maintains proficiency through ongoing professional development.
  • Performs additional duties as assigned.

Qualifications

Required:
  • Active pharmacy license in the State of Illinois.
  • Bachelor's or Doctor of Pharmacy degree.
Preferred:
  • Experience in specialty areas such as neonatology, critical care, or oncology.
  • Completion of a pharmacy residency program or equivalent hospital pharmacy experience.
